Determine Eligibility
Proof of Address
Verification of Age
Items for verification of births; Birth Certificate, Life Certificate, Medi-Cal Card, Baptism Certificate, Passport, or other documentation that confirms the child's age.
Verification of Income
To determine income eligibility parents may use one of the following:
• Prior years income tax returns
• Prior years W-2 Forms
• Pay-stubs reflecting 12 months of income for relevant time period
• Current CalWORKs Verification
• Social Security Income
• Foster Care Assistance
• Declaration of Homelessness
